Knee Pain

Patient :77-year-old woman

 

The right knee was edematous and painful to walk; the knee had been pumped once and there was still no improvement.

 

Go to the hospital to check the X-ray, the doctor suggested surgery as soon as possible, because she is afraid of surgery, the daughter's idea is not want to mother surgery.

 

Question:

 

The fascia of the leg is inflamed and tight, and the thigh bones and the thigh bones are pulled close to each other and squeezed. After no improvement, it is more and more tight, resulting in the compression of the tissues in the knee, the exudation of tissue fluid, and the body cannot metabolize too much tissue fluid, resulting in knee hydrops.

 

The root cause of "knee edema" generally has three: lumbar spine, hip or poor walking posture.

 

After carefully pressing and testing several parts, it was found that the crux of the problem was "lumbar reverse arch" which caused "sciatica". "Sciatica" (sciatic fasciitis) made the foot tendon tight and short, thus pulling the thigh bone and the thigh bone to squeeze each other. The fascia tights from the lumbar spine all the way to the lower leg, also causing weakness in the feet. So, the sites of severe fascial inflammation are ~

 

1) Three groups of lumbar fascia

2) Seven groups of fascia of intestinal (iliac) bone

3) Three groups of fascias of the iliac joint

4) Group 3 fascia of ischium

5) Three groups of hip fascia

6) Seven groups of fascias

 

Processing method:

 

After the tendons and tendons are massaged on all the relevant fascia, the pressure of the fascia pulling on the thigh and thigh bones immediately dissipates, and the knee swelling is significantly improved.

🖐️ Ape Hand Deformity (Median Nerve Palsy)

Ape hand deformity is a characteristic hand posture seen in median nerve palsy, where the thumb loses opposition and abduction, resulting in it lying in the same plane as the fingers.

🔬 Pathophysiology

The median nerve supplies:  Thenar muscles (APB, OP, FPB)

Injury → thenar muscle paralysis

Leads to:  

1️⃣ Loss of thumb opposition, 

2️⃣Loss of abduction,  

3️⃣Unopposed action of adductor pollicis (ulnar nerve)

👉 Result: Thumb appears flattened and adducted, resembling an ape’s hand.

⚠️ Causes

👉Wrist-level injury (most common)

👉Carpal Tunnel Syndrome

👉Forearm trauma

👉Iatrogenic (surgical injury)

👀 Clinical Features

1️⃣ Thenar eminence wasting

2️⃣ Thumb:  Adducted,  Laterally rotated,  Cannot oppose little finger

3️⃣Weak precision grip (pinch impaired)

🧪 Clinical Tests

✅Opposition test → patient unable to touch thumb to little finger

✅Pen test → difficulty holding pen between thumb and index

✅“OK sign” weakness (in high lesions)

🛠️ Management

Hamstrings feel tight

If your hamstrings constantly feel tight…
but stretching gives little or no relief, the source may not be the muscle.

👉 It may be lumbar nerve root irritation (commonly L5)—often associated with L4–L5 spinal stenosis or disc pathology.

At the L4–L5 level, structural changes such as:

Disc bulge or degeneration

Ligament thickening

Facet joint enlargement

can narrow the spinal canal or foramina, leading to:

👉 Compression or irritation of nerve roots

This can produce symptoms along the sciatic distribution

⚠️ Why It Feels Like “Tight Hamstrings”

Nerve-related pain can mimic muscle tightness due to:

⚡ Altered neural signaling (perceived as tightness)
